Yavneh Day School Back-To-School Parent Checklist for the 2024-2025 School Year:
Newly Enrolled Students:
GradeLink: Complete Registration by 8/9/24
(follow instructions to reset/create password)
Kindergarten:
- Submit Your child’s Immunization Records
- Your child’s Health Examination form: Please make an appointment with your child(ren)’s physician to receive the required vaccinations and have the physician complete the Health Examination Form.
- TB-Risk-Assessment: Please make an appointment with your child(ren)’s physician to complete the TB Risk Assessment.
Grades 1st – 8th:
- Your Child’s Release of Records
- Submit Your child’s Immunization Records
** ALL immunization records can be emailed to [email protected] or dropped off at the front office**
Returning Students:
GradeLink: Review/Update Your Registration by 8/9/24
All Students Entering 7th Grade:
- Tdap and Varicella immunization: All students entering 7th Grade must provide documentation showing Tdap vaccination (whooping cough) booster shot given on or after 7 years old AND 2 doses of Varicella (Chickenpox) vaccine.

All Families:

- Submit Medical Authorization Form if your student requires medication at school
- Cafe Permission Slip (Grades 6-8)
- Walk Out Permission Slip (Grades 4 and up)

- Pack (2) Extra Kippot or Head Coverings
- Pack Emergency Comfort Kit: Provide a gallon-size ziploc with your child’s name, change of clothes, several healthy snacks (crackers, pretzels, granola bar, dried fruit), a beverage (juice box or small water bottle), and a short note as a comfort provision in the unlikely event of an emergency.
